Physician Service Reports

Save your hospital time and money.
The Indiana Hospital Association has a data service that makes the credentialing process more efficient. IHA Physician Service Reports will reduce the amount of manual data discovery required by hospital staff and physicians. This service could save your staff hundreds of hours. The report provides a surgeon’s total procedure volume in Indiana. Physician reports are generated from IHA’s inpatient and outpatient surgery databases.

These statewide databases contain patient-level information for all discharges from Indiana acute care hospitals. Data on ambulatory procedures from hospitals and hospital-owned clinics is also captured.

The annual fee is based on the number of physicians.
Individual physician information is provided in PDF and/or Excel format. The annual service fee is calculated on a volume scale, based on the total number of physician identification numbers submitted to IHA. The scale is as follows:

Release Policy
Participating hospitals are required to sign a release policy acknowledging that:

  • Data provided to IHA remains the exclusive property of the originating hospital.
  • IHA makes no guarantees regarding the accuracy and completeness of the data provided to IHA by its members.
  • Information distributed by IHA may only be used for purposes of quality, credentialing, or privileging activities pertaining to the requesting hospital.
  • Hospitals are encouraged to make information available to the physician for purposes of obtaining the physician’s confirmation of the information’s accuracy and/or completeness.
