HRET is challenging every hospital to interview at least five patients who were re-admitted within 30 days by Oct. 1. During the July 5 state partner call, HRET reviewed the following resources from the all-HIIN meeting at the end of June.
- Readmission Review Tool - use this tool to better understand the human side of readmissions root causes.
- Hospital Inventory Tool and Community Inventory Tool - use these tools to identify activities to leverage across both the hospital and community settings.

Please use these tools to conduct five patient readmissions interviews. Ask the hospitals to give you a summary of key themes and include these key themes in your state monthly report.
