The August 22 webinar will feature speakers from CarDon & Associates and Parkview Health discussing senior care and keeping extended-living readmission rates low. CarDon & Associates will be discussing the heart failure initiative they have in place and the innovative "Follow Me Home" pharmaceutical program. Parkview Health will be detailing the Sepsis pilot program they have implemented and the results from the study. A brief overview of the sepsis pilot can be found below.
Parkview's Community Paramedical in SNF’s: Started May 2016
Residents in long-term care (LTC) facilities can be treated for early signs of sepsis in the facility using early warning signs detection skills. Early treatment protocols significantly decreased the frequency of resident’s being transported to an acute care facility. A community paramedic rapid response team has been developed to intervene when LTC residents have a deterioration in clinical symptoms. Clinical quality is advanced as represented by the outcomes of early detection of sepsis and development of facility treatment protocols. The community paramedic rapid response program has decreased residents being transported to acute care facilities for care that can be provided in the LTC, decreased hospital readmissions, and improved advance care decisions.
