Special Innovation Project: Advancing the Health of Hoosiers in Rural Indiana (SIP)

Hospitals will begin by developing implementation strategies to address the significant community health needs identified in a Community Health Needs Assessment (CHNA). A CHNA process can focus an organization’s efforts around community health improvement and provide structure for addressing the determinants of health and illness in a community. *This is a special, 4-year project. ​​​​​
