ISDH Releases 2016 Medical Error Report

​​The Indiana State Department of Health (ISDH) has released the 2016 Medical Error Report. Statewide, the total number of reported adverse events in 2016 was 127, with 122 reported by hospitals. This year has the most reported events for a year since the beginning of medical errors reporting in 2006, with 127 reported. The most reported event for 2016 was stage 3 or 4 pressure ulcers (also known as bed sores) acquired after admission. The second most reported event for 2016 was surgery performed on the wrong body part.  ​​

Be sure to check the data table Appendix B for your individual hospital report on ISDH’s website. The data tables provide the number of inpatient discharges, the number of outpatient visits, and the number of procedures performed by each hospital for 2016.
