Enrolled Act 1145 passed in 2017 regarding a statewide stroke network requires
Indiana hospitals certified as comprehensive stroke centers, primary stroke
centers, or acute stroke ready hospitals to submit a copy of their
certification to the Indiana State Department of Health (ISDH) by July 1. In
addition, if your hospital is not certified but participates in a stroke
network, you must provide a copy of the hospital’s written transfer agreement
with a comprehensive, primary, or acute stroke ready hospital. Send
