Medications are the most common source of adverse events in hospitalized patients. The American Hospital Association advises that serious adverse events are limited to a few medications. CMSA/IHA Anticoagulation Safety Toolkit

CMSA finalized the Anticoagulation Safety Toolkit that provides materials from the 2013 ADE webinar series. The recordings and slides of past ADE webinars are available on the IHA website under ADE Archived Webinars
