Healthcare Advisors, QSource, and IHA for one of our regional collaborative
meetings for health care providers to discuss how we can work together to
improve care transitions. The same event will be hosted four times in four
different regions to make the event accessible to attendees from around the
state. All events will be hosted from 1 - 5 p.m. of the local time zone. Download
