Per a recent bulletin, Indiana has revised the State Plan Amendment to exclude 340B drugs dispensed by Contract Pharmacies. IHA has confirmed with the state that this revision applies only to Fee for Service Medicaid populations and does not apply to managed Medicaid patients.​
